Project Description:
CARE's activities proposed Community REACH project will contribute to global efforts to prevent HIV/AIDS infection, as well as to provide care and support for people who are infected and affected under the USAID Washington Intermediate Result 4.4 : Strengthened and expanded private sector organizations responses in delivering HIV/AIDS information and services, as well as USAID-Rwanda Intermediate Result 2.1 reads: Increased availability of decentralized, quality primary health care and STI/HIV services in targeted regions.

Under these objectives, CARE has four (4) project objectives:
1. Improved quality of basic home care and psychosocial support services provided by partners to PLWA
2. Enhanced capacity of community institutions to provide support and care services to orphans and vulnerable children
3. Establish referral mechanisms with local legal institutions, health centers, clinics and hospitals for legal services, prophylaxis, treatment of opportunistic infection, and palliative care
4. Improved knowledge about, and scope of information channels to address obstacles in behaviors and practices to prevention and mitigation of HIV infection

Through this project, the expected results include:
Increased utilization of basic home care and psychosocial support services by PLWA
Increased utilization of community services by orphans and vulnerable children
Strengthened referral mechanism with local legal services, health centers, clinics and hospitals
Increased integration of HIV/AIDS prevention messages and support actions for PLWA by community groups and institutions.
